Please make the appointment during your headache episode without taking your headache medication, if you mention that you have a headache we will make every effort to schedule an appointment for the same day. If you have the CT scans of your sinuses please bring them with you otherwise do not take a new one because we need a special CT scans.


What is new in Headache and Migraine?

According to American Headache Societies most recent statistics 18% of women and 6% of men suffer from Migraines, not including Chronic Daily Headaches, Tension Type Headaches and Cluster headaches. Migraine disease is subdivided into 2 categories. There is Migraine with Aura (a blind spot in the eye with flickering lights around it that moves away and proceeds before the headache attacks) and Migraine without Aura.


Recently a subgroup of patients that suffer from migraine headaches without aura (also known as common migraine), Chronic Daily Headache and Tension Type Headaches, were found to have contact points inside their nasal cavity. Treating these patients with the new outpatient modified sinus surgery (removing the contact point areas) showed that the majority of patients were either cured from headaches or had significant improvement, translating to quality of life improvement.


Our goal is to assure a good result in order to do so the area of intranasal contact point will be anesthetized during the office visit to check if the headache is improved.


What is Contact Point?

Reviewing the anatomy of the nose shows that there is a space between the septum and medial wall inside the nasal cavity (Fig 1).


When two structures inside the nasal cavity press against each other (Fig 2), in some individuals (people who suffer from migraine and other chronic headaches) this abnormality will facilitate continuation and exasperation of pain in the head and neck area that makes a loop of a vicious cycle. By removing one of these structures we are breaking this cycle and alleviating or reducing the headaches (Fig 3,4).


The operation is called "septoplasty, turbinectomy and modified ethmoidectomy" (during usual ethmoid surgery the medial wall of ethmoid sinus is not removed, but in this operation the medial wall of ethmoid sinus should be removed). It requires general anesthesia (in some cases when necessary it could be done under local anesthesia), performed at an outpatient facility, and recovery is about ten days to two weeks.
